Counselling consent form
Dear esteemed client,
To establish a mutual understanding of the rights and obligations of both you and our therapist, we refer you to our terms and conditions prior to the commencement of the counselling sessions. By consenting to the contract, you agree to fulfill your duties as outlined in the contract.
How it works
Each counselling session at Ankodi Relationship Therapy lasts 45 minutes. The number of sessions can be extended after regular reviews of the counselling work with the therapist. During the sessions, it is not upon the therapist to provide solutions. The therapist supports you to come up with solutions that address your situation.
1. Place of session
Ankodi provides counselling in person and online, while maintaining privacy and confidentiality. In-person sessions are held at Diani Close, Ole Dume Road, Nairobi, or at a location designated by the therapist. Online sessions are available as an alternative.
2. Contact information
Reach Ankodi on +254 759 382 730 or info@ankodi.org. During counselling you can also contact the therapist on their telephone number or email. Voicemails cannot be left on the therapist’s phone. If you cannot reach them by phone, send an email or use another contact method.
3. Boundaries of contact
We will communicate the therapist’s office hours and contact information to you. In an emergency, follow the emergency protocols specified by the therapist. If the therapist is not available to respond immediately, seek assistance from the appropriate emergency services.
4. Rescheduling and cancellation
To reschedule, contact info@ankodi.org and let us know how you prefer to be contacted. If you are running late, tell us whether you would like us to contact you and by which method. Cancellation needs at least 24 hours’ notice. Rescheduling needs at least 48 hours’ notice if you cannot attend.
5. Payment schedule
The client makes full payment for the initial session in advance, based on the category rate, and a 50% deposit of the sum of the subsequent sessions, so that therapy is not terminated midway. Current rates are on the packages page.
6. Complaints
If you are dissatisfied with any aspect of the service, a formal complaints procedure is available. This agreement is fully understood and agreed to, and stands as a contract for our counselling work together.
